Foster Care and GCM Services
Foster Care is an option for individuals in need of nursing home care. It allows one to live in the community in the most natural setting, with a family in a home, rather than live in an institution for long term care.
If you or your family member is in need of help with personal and/or medical care, CMI can help. We recruit, train and develop foster homes for the elderly and/or disabled. Then we match your needs with their skills and assist with placement. We also provide the ongoing monitoring and care coordination to assure your changing needs are addressed.

A team of Registered Nurse and a Master's Level Social Worker work together to coordinate your medical care and social needs. They provide the following services;
- Continuous assessment and care planning to address each of your medical and social needs.
Monitoring to assure proper implementation of the Personalized Service Plan.
Training for caregivers.
Care coordination.
