We provide solutions and opportunities to those in need of long term care to enjoy the highest quality of life in a family setting. Foster Care is an option for individuals who are unable to live independently in their own homes. Rather than living in an institutional setting, elders remain in the community with a caring family in a loving home.

We believe, and it is proven, that elders respond positively to the unique role they are given when they are placed into a foster home. Their quality of life improves. They may experience better health. They usually become the respected elder in the household as they are cared for by the younger generation. In fact, everyone involved enjoys a rich and rewarding expanded family experience.

A team of Registered Nurses and a Master’s Level Social Workers work together to coordinate your medical care and social needs. They provide the following services;

  • Continuous assessment and care planning to address each of your medical and social needs.
  • Monitoring to assure proper implementation of the Personalized Service Plan.
  • Training for caregivers.
  • Nursing delegation of skilled tasks.
  • Care coordination.
  • Conflict resolution.
  • A geriatric care manager specializes in assisting older people and their families with long-term care arrangements.

 

CMI can help
  • Conduct care-planning assessments to identify problems, eligibility for assistance, and need for services;
  • Screen, arrange, and monitor all services identified in the plan of care;
  • Act as a liaison to families at a distance, making sure things are going well and alerting families to problems;
  • Provide information and referral to those exploring their long term care options.
  • Other counseling and support; and more.
  • CMI Case Managers have extensive knowledge about the costs, quality, and availability of services in their community.
  • One call to CMI will connect you with the services you or your loved one may need.

 
What we can do for you

If you or your family member is in need of help with personal and/or medical care, CMI can help. We recruit, train and develop foster homes for the elderly and/or disabled. Then we match your needs with their skills and assist with placement. We also provide the ongoing monitoring and care coordination to assure your changing needs are addressed.
